Authorities rushed a young family and middle-aged man to hospital on Friday evening, after a two-car collision near the Pinjarra and Lake Road intersection.
About 8.30pm on December 28, emergency services were called to the scene that left carnage scattered across the road and median stripe.
It will be alleged the 58-year-old man was driving a black Volvo XC60, west along Pinjarra Road when he collided with the rear of a black Toyota Prado, which contained two adults and their toddler.
As a result of the crash, the Volvo left the road and ploughed through a fence on the southern side of the dual carriageway.
Both car sustained significant damage in the incident.

A small section of Pinjarra Road, between Lakes and Old Pinjarra Road, was closed by police for about an hour immediately after the incident, forcing motorists travelling in the area to take a detour.
Department of Fire and Emergency Services crews were on site to free the man and help clear the scene.
Several St John Ambulances also attended to treat and transfer the occupants.
The 58-year-old sustained broken bones and was taken to Royal Perth Hospital.
A 29-year-old woman and the toddler in the Toyota sustained minor injuries and were treated at Peel Health Campus.
A man at the scene told the Mandurah Mail the man was seen shouting and required several people to contain and lift him onto a stretcher before paramedics could treat his injuries.
The crash marks the third in the city on Friday, after a car ploughed through a brick wall along Mandurah Terrace and two sedans collided at the Aldgate and France Street intersection.
