A worker has died at BHP Billiton's Worsley alumina refinery in Western Australia's south west.
A Department of Mines and Petroleum spokeswoman said the company notified it of the fatality late on Monday afternoon.
In a statement, a BHP spokesman confirmed the death, which occured at the refinery's powerhouse.
"A full investigation is underway," he said.
"We extend out deepest condolences and support to the deceased's family, friends and colleagues at this difficult time."
The department is investigating the circumstances surrounding the death.
It is the third accidental workplace death in a matter of days after a 27-year-old man was struck by a piece of machinery at a Karratha work site and a man aged in his 40s was crushed by a beam in an accident in Kewdale on Thursday.
The man later died at Royal Perth Hospital.
